Special Considerations When Making An Appointment

We are here to serve your holistic care needs along with your gynecology, dermatology and family practice needs—and much more!

Ohio Holistic Healthcare is a personalized office-based practice in Amherst located at 570 North Leavitt Road. Our physicians are local and have been serving Lorain county for many years. Dr. Kovach has been a beloved OB/GYN in the community for over 20 years and Dr. Dorobek has been an extraordinary Family Practitioner with a special interest in dermatology for about the same. Their passion for patient care is unmatched. Currently, Ohio Holistic Healthcare is a direct payment office. This means that we accept cash or credit cards for payment. If the service you are being seen for is covered by your insurance, then we will give you the necessary documentation needed to submit a claim to your insurance company for subsequent reimbursement. Our gynecology, minor office surgical procedures, family medicine, and dermatology services are generally eligible for insurance reimbursement depending upon your plan and deductible. You should call your insurance to check your coverage for services like acupuncture, reiki, massotherapy, and cosmetic procedures. The most advantageous for patient is the utilization of HSA, HRA, and FSA accounts to pay for the above services. (Please refer to health talk article form last week for more details on these flexible and tax-saving payment options.)

Medical Marijuana is not covered by any insurance company at this time. This means you may not submit to your insurance company for reimbursement. The office visit at OHHC for a new patient appointment is $250.00. Any visits related to medical marijuana after your initial appointment will be $150.00. There is unlimited educational support for our cannabis patients offered through our office. Prior to making an appointment at our office, please visit our website: www.ohioholistichealthcare.com. This is where you will find valuable information and updates regarding our office, including the forms needed at your first appointment. Please make sure that you print the intake form and fill it out entirely prior to your appointment. There is also a records release form and an attending physician’s form. If the doctor treating you for your qualifying medical condition is willing to fill out the attending physician’s attestation form, please have them do so and bring this to your first appointment. If not, please fill out the records release form and have the respective doctor’s office that treats your qualifying medical condition fax your records to our office at 440-370-3026. Once you have requested your records to be faxed to us, please call us at 440-340-1970 to schedule an appointment. It is very important that we have records at the time of your appointment with us to ensure that we can get your information in to the Medical Marijuana Registry as quickly as possible.

Please note that a working email is required by the state of Ohio. This email may belong to your or your caregiver. Email is how they will communicate with you and send you the link for your medical marijuana card. The state of Ohio will also charge you a $50.00 registration fee for the card.

The 21 qualifying medical conditions eligible for treatment with medical marijuana in the state of Ohio are as follows:

  • POSITIVE STATUS FOR HIV

  • ACQUIRED IMMUNE DEFICIENCY SYNDROME

  • ALZHEIMER’S DISEASE

  • AMYOTROPHIC LATERAL SCLEROSIS (ALS)

  • CANCER

  • CHRONIC TRAUMATIC ENCEPHALOPATHY (CTE)

  • INFLAMMATORY BOWEL DISEASE

  • CROHN’S DISEASE

  • ULCERATIVE COLITIS

  • EPILEPSY OR ANOTHER SEIZURE DISORDER

  • FIBROMYALGIA

  • GLAUCOMA

  • HEPATITIS C

  • MULTIPLE SCLEROSIS

  • PAIN THAT IS CHRONIC AND SEVERE OR INTRACTABLE

  • PARKINSON’S DISEASE

  • POST-TRAUMATIC STRESS DISORDER (PTSD)

  • SICKLE CELL ANEMIA

  • SPINAL CORD DISEASE OR INJURY

  • TOURETTE’S SYNDROME

  • TRAUMATIC BRAIN INJURY (TBI)
